Together we can do more.   We are seeking an experienced Clinical Nurse Specialist to join our Aged Care Outreach Service team. As the successful candidate, you will play a key role in providing timely, high-quality care to residents in Residential Aged Care Facilities (RACFs). The role will focus on offering acute care treatments at the residents’ homes, enabling them to remain in their familiar surroundings while receiving the best possible care.   Your responsibilities will include assessing, diagnosing, and managing acute deteriorations in RACF residents, reducing emergency department presentations, and promoting hospital avoidance strategies to minimise complications. In addition, you will actively contribute to upskilling RACF nursing staff through education on common aged care conditions and the development of care pathways for recognising and responding to acute deteriorations.
