Community Mental Health Clinician, Care Coordination Team

About this role

We are looking for a motivated mental health clinician to work Part Time in our Community Mental Health Rehabilitation Team. Duties will include recovery oriented practice based assessments, collaborative goal/ treatment planning/ provision, and care coordination for allocated consumers and assisting with the development, implementation & evaluation of the DBT program. • Current AHPRA registration for relevant discipline - Psychology, Occupational Therapist, Registered Nurse, Eligibility for membership with AASW for Social Workers. • Demonstrated experience in comprehensive mental health assessment and biopsychosical interventions including DBT. • Current NSW Driver's licence.
