Older Persons, Subacute and Rehabilitation Services provide a range of inpatient and community services focussing on interprofessional care, integration of services, preservation and restoration of health and vitality in managing complex health conditions, system navigation and care planning

About your opportunity:

In this position you will…

• Foster an organisational culture that supports an unwavering focus on clinical safety, quality, risk and continuous improvement.

• Work cooperatively and effectively by maintaining timely and open respectful communication (including accurate documentation and reporting) within multi-professional teams, with patients, their families/carers and other health professionals, supervisors, students and researchers as required.

• Provide physiotherapy leadership and contribute to daily clinical management of the subacute physiotherapy team.

• Maintain professional links both within the CHHHS and with relevant external clinicians. This may include regular involvement with the Geriatric and Rehabilitation State Wide Professional Networks.
