The Community Chronic Care Service is a multidisciplinary service that provides both face-to-face and virtual care in the community including care navigation, care coordination and health coaching interventions. The Community Chronic Care Service works with Aboriginal and non-Aboriginal people of all ages living in the Illawarra Shoalhaven district with a life limiting chronic illness. The Community Chronic Care Service supports patients to live independently and comfortably at home as long as possible with access to allied health and nursing staff and remote monitoring of their symptoms and wellbeing. This position is part of the Community Chronic Care Service multidisciplinary team and provides clinical supervision to Level 1-2 social work staff and university students. The position will provide specialist social work service to Community Chronic Care Service patients and their families and carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation and the NSW Health Code of Conduct. The Social Worker is responsible for the provision of specialist services, clinical leadership, guidance and supervision of social work staff in Community Chronic Care Service as well as carrying a specialist clinical caseload. The position will increase access to end-of-life psycho-social services for people with a life limiting chronic illness. This may include psychosocial skills in assessing and assisting the management of patients and families with degenerative conditions and patients requiring acute care following discharge from hospital. The Social Worker will provide support with service navigation issues and advance care planning. The Social Worker will deliver care through multiple methods including telehealth, virtual communications and face-to-face sessions.
