As the Aboriginal Health Practitioner, you will be central to the delivery of coordinated, culturally safe, and patient-centred care that bridges hospital, virtual, and community-based services. This role supports the delivery of virtual care to enhance equity, access, patient flow, and service connection across the region. This role forms a key part of the Western Virtual Connected Care in the Community for Aboriginal people - Planned Care for Better Health program. Your contribution will support the delivery of holistic, person-centred care that makes a meaningful difference to the health and wellbeing of Aboriginal people, families, and communities across Western NSW.
