About the Role Formed in 1882, Royal Prince Alfred Hospital (RPAH) is a major metropolitan tertiary referral hospital and a principal provider of specialist healthcare. It remains one of the nation’s most respected hospitals, with a distinguished history serving the health needs of local, statewide, national and international communities. The Planned Care service delivers short term (up to 12 weeks), innovative, integrated care that supports people with complex social and medical needs to live well in the community. The PCBH service targets adults who are at risk of hospitalisation and aims to reduce avoidable hospital presentations and duplication of services.
The Care Coordinators work within a multidisciplinary community-based team that delivers individualised support and coordination of a person’s health and social support needs. The service aims to improve overall continuity of care and connectivity between primary, hospital and community health providers. For more information, view the Position Description Ideal Candidate • Tertiary qualifications in Nursing or Allied Health (Specifically Social Work, Occupational Therapy, Physiotherapy or Speech Pathology) with AHPRA or eligibility for membership to appropriate Professional Associations where required.
• Experience working with adults in their homes or demonstrate knowledge about managing clients in a community setting with complex needs, including comprehensive assessment and care coordination or case management using sound clinical reasoning.
• Current NSW Drivers Licence (P1 and P2 acceptable).
