The HITH team currently has vacancy for both Permanent positions and Temporary positions. About the Opportunity As a Hospital in the Home Registered Nurse, you will provide comprehensive clinical care to patients across the Central Coast community. You will assess, plan, implement and evaluate patient care while working closely with medical specialists, GPs, allied health professionals, carers and community services to deliver seamless healthcare experiences. This position plays a key role in delivering high-quality, hospital-level care to patients in their own homes or alternative community setting, promoting independence while improving health outcomes across the community. In this role you will: Provide high-quality, patient-centred nursing care in patients' homes across the community. Conduct clinical assessments, develop care plans, and deliver evidence-based treatments. Collaborate with multidisciplinary teams to coordinate safe, seamless patient care. Educate and support patients and their carers, promoting independence and positive health outcomes. For more information about this role, please view the Position Description About You We are looking for someone who has: Current registration as a Registered Nurse (Division 1) with AHPRA and a current Australian Driver Licence. Recent experience in acute care nursing, or a strong interest in developing their acute care expertise. Excellent clinical assessment, care planning and case management skills. Strong communication and interpersonal skills, with the ability to work both autonomously and collaboratively within a multidisciplinary team. A commitment to evidence-based practice, continuous improvement and delivering exceptional patient-centred care.
