What you'll be doing To provide expert clinical leadership and coordination of comprehensive Palliative and End of Life Care Services across a defined geographical area. The CNS2 – Coordinator is responsible for the daily triage, allocation, intake, and clinical oversight of palliative care patients, supporting multidisciplinary service delivery, ensuring timely access to high quality, evidence-based care, and promoting clinical mentorship, continuous service improvement, and person-centred practice. About You a { text-decoration: none; color: #464feb; } tr th, tr td { border: 1px solid #e6e6e6; } tr th { background-color: #f5f5f5; } As a Registered Nurse with current registration through the Nursing and Midwifery Board of Australia, you bring extensive specialist palliative care expertise supported by a relevant postgraduate qualification and more than three years of recent clinical experience. You demonstrate advanced clinical assessment capabilities, including the management of complex patient presentations, and are highly skilled in completing and interpreting Palliative Care Outcome Collaboration (PCOC) assessments to guide clinical decision-making, triage, care planning, and ongoing patient management. You are a confident clinical leader who is committed to fostering excellence in care through mentoring, educating, and supporting multidisciplinary team members. Your experience in service coordination, caseload management, and triage enables you to effectively prioritise patient needs while ensuring the delivery of high-quality, person-centred palliative care services. You are recognised for your strong communication, negotiation, and interpersonal skills, with the ability to build effective relationships across all levels of healthcare and work collaboratively with patients, families, carers, and multidisciplinary teams. Dedicated to continuous professional development and quality improvement, you actively utilise clinical data and evidence-based practice to inform service development and enhance patient outcomes. You also hold a current unrestricted NSW Driver’s Licence and are able to travel across the Mid North Coast Local Health District to support service delivery as required.
