This role sits within a multidisciplinary team committed to improving health outcomes for Pacific communities through culturally sensitive and community-driven approaches. · Work in a supportive, inclusive and culturally aware environment · Collaborate with health professionals and community leaders · Be part of a service that values cultural identity and lived experience · Access professional development and clinical supervision · Make a meaningful impact across multiple WSLHD sites What you'll be doing As a Pacific Health Worker, you’ll play a vital role in bridging the gap between Pacific communities and health services, delivering tailored support and education that promotes wellbeing and access to care. · Provide assertive case management, advocacy, and health education · Develop and deliver culturally appropriate health programs and resources · Facilitate referrals and transport assistance to improve service access · Build strong partnerships with local agencies and community groups · Support community development initiatives and early intervention strategies How to Apply Apply online by submitting your resume and addressing the criteria listed below by providing examples that demonstrate your ability before the closing date. Essential Criteria: · License: Driver's licence valid in NSW · Qualification: HEO Graduate/non graduate - Relevant qualification in social sciences, health or related field Selection Criteria: 1. Relevant qualifications in social sciences, health or related field – to be considered under graduate classification. 2. Demonstrated knowledge and understanding of Pacific communities and have established links with Pacific communities or demonstrated capacity to build linkages. 3. Demonstrated understanding of health issues that affect Pacific communities and strategies to improve health outcomes for Pacific people. 4. Demonstrated knowledge and / or experience in developing, implementing, reviewing, and evaluating culturally appropriate health education / health promotion / community development programs and initiatives to Pacific communities. 5. Demonstrated experience and/or abilities in building and maintaining successful partnerships with relevant partners including government and non-government organisations and community based organisations. 6. Demonstrated ability to work autonomously and as a member of a multidisciplinary team with excellent organisational, written and verbal communication skills 7. Demonstrated computer skills with experience in a range of Microsoft applications. 8. Current Drivers Licence valid within New South Wales and the willingness and ability to travel.
