An exciting opportunity for a Registered Nurse – Hospital in the Home (HITH). Where you'll be working HITH NSLHD (Between Hornsby, RNSH and Mona Vale sites) What you'll be doing Assess, plan, implement and evaluate community nursing care, in the patient home or clinic, in collaboration with individual/s and the greater health care team to achieve quality patient outcomes. People of Aboriginal and/or Torres Strait Islander background are encouraged to apply. Candidates will need to meet the following criteria: * Registered Nurse –Division 1 with Australian Health Practitioner Regulation Agency (AHPRA) and relevant nursing experience. * Demonstrated ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers. * Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.
