Sydney Local Health District is one of the fastest growing Local Health Districts in New South Wales. Our facilities and services are world class and our staff are dedicated, innovative and caring.   About the Role   The Acute Care Service model of care provides;

1. Mental Health assessments to people experiencing acute mental health problems ( including a rapid response when indicated) and determining an appropriate pathway to care (eg community treatment; assist consumers access  and admission to inpatient treatment)  2. Short term intensive treatment in the community often as an alternative to hospital  3. In reach to hospital to promote effective follow up post mental health admission  4. Gatekeeping- developing opportunities for assessment and community treatment at key points (eg; Assessment Unit and Admission Office)  5. The model provides short term treatment after assessment up until the acute symptoms are resolved or the consumer is effectively transferred and engaged with an appropriate treatment provider.

The clinician will practice in keeping with the standardised Sydney LHD position description for a Registered Nurse in addition to this position description.
