About the Role: The Palliative Care Social Worker will provide support for people with a life-limiting condition and their families within Murrumbidgee Local Health District. The Social Worker is an integral part of the MLHD Specialist Palliative Care Team, working alongside patient liaison nurses to provide multidisciplinary social work input, enhance early palliative care services, and ease transition of care.
