About the Opportunity As a Registered Nurse within the Care Coordination Team, you will play a pivotal role in supporting consumers throughout their mental health recovery journey. Working alongside Psychologists, Social Workers and other experienced clinicians, you will provide comprehensive assessments, care coordination, treatment planning and therapeutic interventions that make a genuine impact on people's lives. This position plays a key role in delivering high-quality, recovery-oriented mental health care by coordinating comprehensive, person-centred support for consumers with moderate to severe mental health needs. In this role you will: • Conduct comprehensive mental health assessments and deliver recovery-focused, evidence-based nursing care. • Develop and coordinate person-centred care plans in partnership with consumers, families, GPs and multidisciplinary teams. • Provide care coordination and therapeutic interventions that support positive mental health outcomes and community wellbeing. • Collaborate with and mentor colleagues while contributing to continuous improvement and best-practice mental health service delivery. For more information about this role, please view the Position Description About You We are looking for someone who has: • Current registration with AHPRA as a Registered Nurse, supported by a Bachelor/Degree in Nursing and a current Australian Driver Licence. • Demonstrated experience in mental health nursing, with the ability to deliver recovery-oriented, evidence-based care across a range of clinical settings. • Strong communication and interpersonal skills, with the ability to build effective partnerships with consumers, families, carers, GPs and multidisciplinary teams. • Excellent clinical assessment, care planning and case management skills, with the ability to manage complex caseloads and make sound clinical decisions. • A commitment to professional excellence, including ongoing professional development, person-centred care and the values of NSW Health and Central Coast Local Health District.
